Psoriasis is a systemic multifaceted disease that takes many forms and has a broad clinical spectrum. Patients affected with psoriasis and psoriatic arthritis can be found, and treated, by both dermatologist and rheumatologists depending on symptoms. To secure for patients to be optimally treated it is important to be aware of the different manifestations and when to refer to the other specialty.
Axial psoriatic arthritis and back pain
Psoriatic arthritis is a multifaceted disease showing different manifestations in different patients. It is important to keep in mind that the disease may affect not only peripheral joints and skin, but may also give rise to enthesitis, dactylitis, nail psoriasis, and axial disease. Of note, these different disease manifestations show varying responses to different types of immunomodulatory treatment.
Axial disease in psoriatic arthritis, affecting the pelvic and spinal joints and entheses, is reported to affect 25-70% of patients with psoriatic arthritis and may result in inflammatory back pain. Although axial psoriatic arthritis share several features with other axial spondyloarthritides there are also differences.
